To meet the Transitions Service needs to employment or skills development/post secondary education of young adults with disabilities in Prince Albert and rural communities.
Individualized transition action plan service to assist young adults with disabilities in preparing for the transition from high school to adult life. This might include future goals for employment, post secondary education, finding a place to live, being involved in the life of the community, personal growth, and other adult objectives for a desirable future.
An individual must self-identify as having a disability, be unemployed, be legally entitled to work in canada, and be in need of transition services. Participants must not be attending secondary school at more than 20 hours per week and not be eligible for Employment Insurance.
Building self-confidence, making informed decisions, receiving coordination and integrated services, achieving self-advocacy and self-determination, gaining respect for individual choices, developing collaborative relationships, receiving ongoing support and follow up services, accessing other resources and community service agencies, requesting referrals to enhanced supports and planning and reaching your employment or career goal.
